Nurse Practitioner – Full-Time Position
Multi-Specialty Private Physician Group

We are a multi-specialty private physician group seeking a dedicated and motivated Nurse Practitioner to join our growing medical team on a full-time basis.

This is an excellent opportunity for a provider who values quality patient care, collaboration, and work-life balance in a supportive, physician-led environment.

Qualifications:

  • Active New York State Nurse Practitioner license required

  • Family Medicine, Internal Medicine, Geriatrics, Rehabilitation, or Post-Acute Care experience preferred

  • Previous experience in skilled nursing, rehabilitation, long-term care, or assisted living is a plus

  • Strong communication, clinical judgment, and organizational skills

  • Willing to train the right candidate

Responsibilities:

  • Coordinate patient care with attending physicians, families, nursing staff, and facility teams

  • Communicate regularly with physicians regarding changes in patient status and treatment plans

  • Support the medical team in providing high-quality, patient-centered care

  • Participate in care coordination, documentation, and clinical follow-up

  • Help ensure continuity of care across post-acute and long-term care settings
