We are currently seeking an experienced:

HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER

Full Time

(Covering Bladen, Columbus, Robeson)

Job Description:

  • Implements palliative care program requirements
  • Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated
  • Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients
  • Provide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical Director
  • Communicate with patients/families at all levels.
  • Communicate with all agency personnel

Job Requirements:

  • Master’s degree in Nursing required, with current licensure in the state of practice
  • Certification as an Adult or Family Nurse Practitioner
  • Experience in Hospice or Palliative Care nursing preferred
  • Knowledge of the framework, function, and organization of a hospice care program.
  • Excellent communication and organization skills.
