A Licensed Vocational Nurse (LVN) who, under the supervision of the Clinical Manager, coordinates the timely and accurate flow of clinical information during the referral process. The Home Health Intake Liaison serves as a key point of contact between referral sources, patients, and the agency. This role is responsible for supporting the intake process, coordinating referrals, verifying eligibility, and building strong relationships with hospitals, physicians, and community partners to support census growth and smooth patient transitions into home health services.

Required Experience

Current California LVN license
High school diploma
Current BLS issued by the American Heart Association
Current California driver's license and valid auto insurance
Minimum one-year experience as an LVN
Familiarity with EMR systems and basic medical terminology

Preferred:

Home Health or Hospice experience
Associate or bachelor’s degree in healthcare or related field
Knowledge of Medicare, Medicaid, and commercial insurance guidelines strongly preferred
