Job Summary

  • The Transfer Center Access Nurse serves as the primary liaison for transferring patients to a higher level of care facility either by request of the physician (HLOC Transfer) or patient/family (Lateral Transfer). This role involves coordinating the exchange of medical information, arranging transportation, and ensuring all required documentation is completed. The nurse will also provide support within the Transfer Center, including answering calls and facilitating patient placements.

 

Essential Functions

  • Serve as the primary contact for interfacility patient transfers.
  • Coordinate the exchange of patient medical information between facilities.
  • Arrange ground or air transportation for safe patient transfers.
  • Complete all required documentation, including:
    • Memorandum of Transfer
    • Consent to Transfer
    • Reciprocal Agreement
    • PAN documents
  • Assist in the Transfer Center with phone management and patient placement.

 

Educational Requirements

  • Graduate of an accredited School of Nursing (Bachelor of Science in Nursing preferred).
  • Must be able to communicate effectively in English, both verbally and in writing.
  • Certification and Licensure
  • Current, unencumbered Texas Registered Nurse (RN) license in good standing.

 

Knowledge, Skills, and Abilities

  • Minimum two years of recent acute care hospital experience required.
  • Proficient in basic computer navigation and word processing.
  • Strong written and verbal communication skills.
  • Critical thinking and decision-making abilities with attention to detail.
  • Ability to prioritize workload and manage multiple tasks efficiently.
  • Proficiency in electronic tools and documentation systems.
  • Strong interpersonal skills to build positive relationships and effect change.
  • Demonstrates patience, diplomacy, and professionalism in daily interactions. 

 

Physical Requirements

  • Prolonged standing and sitting, with frequent bending, stooping, and stretching.
  • Ability to walk and climb as needed.
  • May require lifting up to 25 pounds.
  • Requires hand-eye coordination and manual dexterity.
  • Ability to distinguish letters and symbols for documentation purposes.
  • Primarily assigned to the day shift, including weekends; flexibility for additional hours as needed.

 

Duties and Responsibilities

  • Determines if transfer request is for higher level of care or a lateral transfer.
  • Coordinate transfers with external Transfer Centers based on patient specialty needs.
  • Arrange safe transportation for patients in collaboration with transport agencies.
  • Ensure all relevant medical records, radiology, lab reports, physician notes, and nursing notes are sent to the receiving facility.
  • Complete and process all required transfer documentation, ensuring proper filing with Medical Records.
  • Evaluate and prioritize transfer requests in coordination with nursing units/staffing.
  • Utilize the Level of Care document to determine appropriate bed assignments.
  • Answer multiple phone lines, responding professionally and courteously to all callers.
  • Identify and address process improvement opportunities within the Transfer Center.
  • Demonstrate flexibility and commitment to departmental needs.
  • Complete special assignments promptly and efficiently.
  • Perform other duties as assigned to support Transfer Center operations.
