Overview
Community Health and Wellness (CHAW) seeks an Occupational Therapist (OT) to provide Therapeutic Consultation under Virginia's Developmental Disabilities Waivers (FIS/CL). OT is a consultative service: assess needs, create a written OT support plan, train and
coach families/caregivers/DSPs, and monitor outcomes. This role does not provide ongoing direct therapy.

Responsibilities
• Conduct OT assessments and observations.
• Develop written OT support plans with strategies/adaptations.
• Demonstrate interventions and train families, caregivers, DSPs.
• Participate in annual ISP meetings and team reviews.
• Complete Medicaid-compliant documentation (notes, reviews, training logs, summaries).

Qualifications
• Occupational Therapist licensed in Virginia
• Experience with individuals with ID/DD.

Schedule & Modality
Unit= 1 hour; in-person or HIPAA-compliant teleconsultation. Frequency determined by
assessed need and ISP authorization.
