Job DetailsJob Location: WILLIAMSPORT, PA 17701Position Type: Full TimeEducation Level: 4 Year DegreeUnder the supervision of the Director of Residential & Community Living Services, the Program Case Administrator (PCA) will manage a caseload of individuals with intellectual disabilities, ensuring personalized support and care coordination. This role involves conducting comprehensive assessments, developing and implementing individualized service plans, and facilitating communication among families, support teams, and external providers. The PCA will monitor progress, advocate for individuals’ needs, and ensure compliance with relevant regulations, fostering an inclusive environment that promotes the well-being and independence of each individual. This position is equivalent to a Program Specialist as detailed in 55 Pa. Code Chapter 6400 from the Office of Developmental Programs. ESSENTIAL FUNCTIONS: The essential functions of this position consist of the ability to perform basic math functions, write, read, and comprehend what is read. Expressive and receptive language skills are also required in the execution of the responsibilities below. The ability to transfer and lift individuals with or without assistance and to occasionally push/pull may also be required. Twisting, bending, and other physical movements may be required at varying degrees. All the above may be performed with or without reasonable accommodation. DUTIES: • Ensure the development, implementation, and review of Individualized Service Plans (ISPs) in collaboration with support teams, conducting comprehensive assessments including annual life medical histories and personal preferences. • Participate in Health Risk Screening Tool (HRST) process. • Complete annual consent and individual rights documentation, ensuring all required paperwork is submitted on time. • Collaborate with the Director of Residential & Community Living Services to evaluate referrals for appropriateness of placement and participate in ISP meetings with Supports Coordinators to discuss individual progress and needs. • Conduct monthly reviews and biweekly in-person check-ins with individuals in residential programs to monitor well-being and support needs and attend home monitoring sessions with Supports Coordinators. • Facilitate effective communication between the support services team including reviewing appointments, attending meetings and providing updates to team members, as needed. • Participate in the preparation of licensing plans of correction and complete Life Safety Inspections within designated timeframes. • Facilitate quarterly check-ins and fire drills for Lifesharing providers. • Performs other related duties and assignments as directed by the Director of Residential & Community Living Services. HOURS: Non-Exempt, 40-Hours per Week IMMEDIATE SUPERVISOR: Director of Residential & Community Living Services POSITIONS SUPERVISED: None Qualifications• Requirements include a master’s degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism OR a bachelor’s degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism OR an associate’s degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism. • Valid driver’s license, required.
