HOSPICE RN CASE MANAGER JOB DESCRIPTION
Position Summary
The RN Case Manager provides clinical leadership and coordination of resident care to ensure services are delivered in accordance with individualized care plans, facility policies, and State/Federal regulations. This role serves as a liaison between residents, families, physicians, and the interdisciplinary team while promoting high-quality, person-centered care and optimal clinical outcomes.
Schedule
- Full-time: 40 hours per week
- Monday–Friday with rotating weekends and holidays
Minimum Qualifications
- Graduate of an accredited School of Nursing
- Current RN license in Minnesota
- Current CPR certification
- Minimum 2 years of long-term care experience preferred
- Strong knowledge of Medicare, State/Federal regulations, Case Mix, and MDS process
- Strong leadership, communication, and organizational skills
- Commitment to resident-centered care and service excellence
Key Responsibilities
Clinical Leadership & Coordination
- Supervise and direct RN, LPN, TMA, and CNA staff to ensure quality resident care
- Monitor clinical outcomes and ensure compliance with care plans and regulations
- Serve as a clinical resource for staff and assist with performance management
Resident Care & Clinical Oversight
- Perform or oversee assessments for changes in resident condition
- Communicate with physicians and implement or transcribe orders as needed
- Support admissions, readmissions, and complex clinical treatments
MDS & Care Planning
- Complete and/or oversee timely and accurate MDS documentation
- Update and maintain individualized care plans
- Participate in care conferences and interdisciplinary team meetings
Quality & Compliance
- Ensure compliance with State/Federal regulations and facility standards
- Participate in quality improvement initiatives and audits
- Identify and report concerns related to resident safety and vulnerable adult issues
Resident Advocacy
- Promote and protect resident rights, dignity, and privacy
- Support effective pain management and person-centered care approaches
