COMPENSATION: The hiring range for this position is $35,000 - $55,000, with the opportunity to earn additional income through a variable compensation (commission-based) structure, offering higher earning potential based on performance.

Position SummaryÂ
Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.Â
Key ResponsibilitiesÂ
- Perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized, evidence-based care plans for residents in a skilled nursing settingÂ
- Provide ongoing medical management including ordering and interpreting diagnostics, prescribing treatments, conducting medication reviews, and proactively monitoring conditions to prevent complications and reduce hospitalizationsÂ
- Deliver timely clinical interventions in response to changes in patient status, ensuring high-quality, patient-centered care that improves outcomes and supports functional well-beingÂ
- Support value-based care initiatives by focusing on quality outcomes, reducing avoidable hospital readmissions, optimizing cost of care, and utilizing data to guide clinical decision-making and performance improvementÂ
- Collaborate closely with physicians and interdisciplinary team members to ensure coordinated, efficient care, and participate in care plan meetings and quality improvement effortsÂ
- Facilitate smooth transitions of care across settings, ensuring continuity, reducing gaps in treatment, and supporting successful admissions and dischargesÂ
- Educate residents and families on diagnoses, treatment plans, medications, and preventative care strategies to promote engagement and long-term healthÂ
- Maintain accurate, timely, and compliant documentation while adhering to all clinical, regulatory, and organizational standardsÂ
- Master’s degree in Nursing from an accredited Nurse Practitioner programÂ
- Active and unrestricted Advanced Practice Nurse (APN) license in respective stateÂ
- Current DEA registration and prescriptive authority (or eligibility)Â
- Board certification as a Nurse Practitioner (e.g., FNP, AGNP)Â
- 2Â years of clinical experience preferred, ideally in geriatrics, long-term care, or post-acute settingsÂ
- Strong clinical assessment, diagnostic, and treatment planning skillsÂ
- Knowledge of chronic disease management and geriatric care principlesÂ
- Ability to deliver high-quality, value-based care focused on outcomes, cost-efficiency, and reduced hospitalizationsÂ
- Proficiency in evidence-based practice and clinical decision-makingÂ
- Ability to work both independently and collaboratively within an interdisciplinary teamÂ
- Excellent communication and interpersonal skills with residents, families, and staffÂ
- Strong organizational, time management, and documentation skillsÂ
- Familiarity with electronic health records (EHR) systemsÂ
