We are seeking a compassionate, collaborative, and clinically skilled Nurse Practitioner to join our team. This role offers the opportunity to build meaningful relationships with participants and caregivers while working alongside an experienced interdisciplinary care team dedicated to improving outcomes for frail older adults.

Position Summary
The Nurse Practitioner provides primary care services to Elderhaus PACE participants through comprehensive assessments, ongoing medical management, acute care interventions, and care coordination. This position plays a vital role in helping participants maintain their highest possible level of health, independence, and quality of life.
As a member of the interdisciplinary team, the Nurse Practitioner collaborates closely with healthcare professionals, caregivers, and community partners to develop and implement individualized plans of care that address each participant's unique needs.

Key Responsibilities
- Conduct comprehensive health assessments and physical examinations for new participants.
- Perform scheduled reassessments and evaluate participant health status as needed.
- Develop, implement, and monitor individualized treatment plans in collaboration with the interdisciplinary team.
- Evaluate and treat acute and chronic health conditions.
- Order, interpret, and follow up on diagnostic tests and laboratory results.
- Prescribe and manage medications in accordance with licensure, privileges, and established protocols.
- Coordinate care with specialists, hospitals, nursing facilities, caregivers, and other healthcare providers.
- Provide participant and caregiver education related to disease management, treatment plans, medications, and preventive health.
- Maintain accurate and timely clinical documentation and diagnostic coding.
- Participate in care planning, quality improvement initiatives, and interdisciplinary team meetings.
- Support staff education and contribute to maintaining high standards of clinical excellence and participant care.

Qualifications
Required
- Current and unrestricted North Carolina Registered Nurse license.
- Certification as a Nurse Practitioner.
- Current CPR certification.
- Valid driver's license, reliable transportation, and ability to meet organizational driving requirements.
- At least one year of experience working with frail older adults or a geriatric population.
- Strong clinical assessment, communication, critical thinking, and care coordination skills.
Preferred
- Prescriptive authority.
- Experience practicing as a Nurse Practitioner in a geriatric, primary care, long-term care, PACE, or related setting.
- Experience working within an interdisciplinary care model.
