This position provides case management services to the Deaf and Hard of Hearing on a regional level. Relies on experience, knowledge, and education with limited direction to provide quality services to the Deaf and Hard of Hearing populations. Master’s Degree in a related field and one year of supervised, post graduated, experience serving persons with mental health or substance abuse: or A Bachelor’s Degree in a Human Service field. Two years of supervised, post graduate, experience serving persons with mental health or substance abuse. Must have knowledge and experience with the age and population served. Be Fluent in sign language, as determined by a Sign Language Proficiency Interview (SLPI) score of Advanced Plus or better. Responsibilities: Provides on-call/first responder services as scheduled at local office. Plans and facilitates consumer transitions to other services and or/local agencies in an efficient manner. Maintains regular contact with the persons served, their treatment teams, guardian and significant others in the persons life’s Assures consistent provision of quality services. Assess persons’ served needs and provides linkages/referrals to those resources which will help them in achieving his/her personal goals Represents RHA at local management entity and other necessary meetings, as related to the care of the persons being served All other duties as assigned. Education/Experience: Master’s Degree in a related field and one year of supervised, post graduated, experience serving persons with mental health or substance abuse: or A Bachelor’s Degree in a Human Service field with two years of supervised, post graduate, experience serving persons with mental health or substance abuse or Bachelor’s Degree in another field with four years of supervised, post graduate, experience serving persons with mental health or substance abuse Must have knowledge and experience with the age and population served. Be Fluent in sign language, as determined by a Sign Language Proficiency Interview (SLPI) score of Advanced Plus or better.
