Now Hiring: MDS Nurse (LPN or RN)


Schedule: 8a-430p


About the Role:

The MDS Nurse coordinates and completes accurate, timely resident assessments in accordance with federal and state regulations. This position works closely with nursing, therapy, social services, dietary, and other members of the interdisciplinary team to ensure each resident’s needs, services, and clinical conditions are accurately reflected in the MDS and care plan.


Key Responsibilities:

  • Complete and coordinate MDS assessments accurately and within required timelines 
  • Develop, review, and update individualized resident care plans 
  • Coordinate care-plan meetings with residents, families, and the interdisciplinary team 
  • Monitor documentation to support resident needs, services provided, and reimbursement 
  • Review clinical records for accuracy, completeness, and regulatory compliance 
  • Track Medicare and managed-care residents, including assessment schedules and required documentation 
  • Participate in utilization review, Triple Check, quality improvement, and compliance activities 
  • Educate nursing staff regarding documentation requirements and changes in resident conditions 
  • Communicate effectively with physicians, nursing leadership, therapy, residents, and families 
  • Assist with audits, surveys, and other regulatory reviews 
  • Maintain current knowledge of MDS, RAI, PDPM, Medicare, Medicaid, and applicable regulations 

Qualifications:

  • Current Kansas LPN or RN license in good standing 
  • Long-term care or skilled nursing experience  
  • MDS and care-planning experience  
  • Knowledge of RAI guidelines, PDPM, and Medicare requirements  
  • Strong organizational, communication, and time-management skills 
  • Ability to meet deadlines and work effectively with an interdisciplinary team
