Now Hiring: MDS Nurse (LPN or RN)
Schedule: 8a-430p
About the Role:
The MDS Nurse coordinates and completes accurate, timely resident assessments in accordance with federal and state regulations. This position works closely with nursing, therapy, social services, dietary, and other members of the interdisciplinary team to ensure each resident’s needs, services, and clinical conditions are accurately reflected in the MDS and care plan.
Key Responsibilities:
- Complete and coordinate MDS assessments accurately and within required timelinesÂ
- Develop, review, and update individualized resident care plansÂ
- Coordinate care-plan meetings with residents, families, and the interdisciplinary teamÂ
- Monitor documentation to support resident needs, services provided, and reimbursementÂ
- Review clinical records for accuracy, completeness, and regulatory complianceÂ
- Track Medicare and managed-care residents, including assessment schedules and required documentationÂ
- Participate in utilization review, Triple Check, quality improvement, and compliance activitiesÂ
- Educate nursing staff regarding documentation requirements and changes in resident conditionsÂ
- Communicate effectively with physicians, nursing leadership, therapy, residents, and familiesÂ
- Assist with audits, surveys, and other regulatory reviewsÂ
- Maintain current knowledge of MDS, RAI, PDPM, Medicare, Medicaid, and applicable regulationsÂ
Qualifications:
- Current Kansas LPN or RN license in good standingÂ
- Long-term care or skilled nursing experience Â
- MDS and care-planning experience Â
- Knowledge of RAI guidelines, PDPM, and Medicare requirements Â
- Strong organizational, communication, and time-management skillsÂ
- Ability to meet deadlines and work effectively with an interdisciplinary teamÂ

Requirements
- High school diploma or equivalent
- Active and valid state-issued Licensed Practical Nurse (LPN) license
- Previous experience as an LPN preferred. New graduates are encouraged to apply!
