Job Purpose & Position Overview: 

  • The purpose of this position is to provide leadership, supervision, coordination, and oversight of the Community Health Worker (CHW) Program within the Winnebago Public Health Department. The CHW Coordinator is responsible for supporting Community Health Workers assigned to specialty program areas including Public Health Nursing, Behavioral Health, Recovery, Maternal Child Health, Diabetes and Chronic Disease Prevention, Senior Services, and other community health initiatives. The CHW Coordinator ensures quality service delivery, promotes culturally grounded and trauma-informed practices, supports workforce development, monitors program outcomes, and strengthens coordination between CHWs, healthcare providers, community programs, and community members.

Specific Job Duties/Responsibilities:

  • Provide direct supervision, leadership, mentoring, and support to Community Health Workers assigned across multiple public health program areas.
  • Coordinate and oversee CHW services within Public Health Nursing, Behavioral Health, Recovery, Maternal Child Health, Diabetes and Chronic Disease Prevention, Senior Services, and other assigned programs.
  • Develop and maintain CHW staffing schedules, work assignments, coverage plans, and program priorities.
  • Conduct regular individual supervision meetings and team meetings with CHW staff.
  • Monitor CHW caseloads, productivity, documentation, service quality, and program outcomes.
  • Ensure CHW activities align with program goals, grant requirements, departmental policies, and best practices.
  • Coordinate referrals and communication between CHWs, healthcare providers, social service agencies, community partners, and internal programs.
  • Assist with development and implementation of standardized CHW workflows, protocols, documentation systems, and quality improvement activities.
  • Provide coaching and support related to home visiting, outreach, care coordination, health education, client engagement, and community resource navigation.
  • Assist with recruitment, onboarding, orientation, training, and retention of Community Health Workers.
  • Complete annual performance evaluations and ongoing performance monitoring for CHW staff.
  • Identify workforce development needs and coordinate continuing education opportunities for CHWs.
  • Coordinate monthly data collection, reporting, performance measures, and grant reporting requirements.
  • Participate in multidisciplinary team meetings, case reviews, care coordination activities, and departmental planning efforts.
  • Support integration of Indigenous ways of knowing, being, healing, and kinship systems throughout CHW service delivery.
  • Promote culturally grounded, trauma-informed, strengths-based, and community-centered approaches to care.
  • Monitor compliance with confidentiality requirements, professional standards, and departmental policies.
  • Assist with development of new CHW specialty positions and expansion of community health worker services.
  • Participate in strategic planning, accreditation activities, quality improvement projects, and workforce development initiatives.
  • Represent the Community Health Worker Program at local, regional, state, and national meetings as assigned.
  • Prepare monthly, quarterly, and annual reports regarding CHW activities, outcomes, workforce development, and program impact.
  • Perform all other duties as assigned.

Qualifications/Skills:

  • Must have a valid driver’s license and meet driver’s eligibility guidelines.
  • Must have and maintain a home telephone/cell phone.
  • Must be competent in Microsoft Office programs – (Word, Excel, Outlook, and RPMS)
  • Two (2) years of college preferably in a Health-related area.
  • At least one (1) year of relevant experience as a supervisor and demonstrated leadership skills.
  • Knowledge of CHR philosophy and responsibility.
  • Must be able to attend initial CHR training sessions in person or online periodically throughout employment.
  • Must be a positive role model in the Winnebago Community.  
  • Knowledge of the principles and practices of healthcare delivery system.
  • Ability to keep strictest confidentiality dealing with all patient records and comply with the Federal Privacy Act.
  • Knowledge of the healthcare resources and services.
  • Knowledge of Medicare and Medicaid eligibility.
  • Knowledge of budgeting, principles and practices.
  • Ability to analyze healthcare problems and services.
  • Ability to keep accurate written reports on a daily, weekly, monthly schedule.

Physical Requirements:  

  • Normal to light office work which includes the ability to lift up to 10 lbs. 
  • Regularly required to walk, stand, and sit for extended periods of time and use hands to operate computer keyboard, phone, and various office equipment. 
  • While performing daily duties, the employee is regularly required to talk and hear.
