Ā Case Manager – Greater Waterville Area

Make a meaningful difference in your community. ā¤ļø We are seeking a compassionate, driven Ā Case Manager to join our team serving individuals and families in the Greater Waterville area. In this role, you'll have the opportunity to support clients on their path to stability, independence, and improved well-being.

What You'll Do

  • Provide client-centered case management services to individuals with behavioral health needs
  • Conduct assessments and develop personalized service plans
  • Coordinate care with providers, community resources, and support networks
  • Advocate for clients to ensure access to appropriate services and supports
  • Maintain accurate documentation in accordance with state and agency standards
  • Support clients in building skills for independent living and recovery

āœ… What We're Looking For

  • High school diploma or equivalent
  • MHRT-C certification (Mental Health Rehabilitation Technician–Community) or MHRT-C Provisional, with the ability to obtain Full MHRT-C
  • Experience working in behavioral health, mental health, or social services required
  • Minimum of one (1) year experience working with individuals with mental illness and/or developmental disabilities
  • Strong communication, organization, and problem-solving skills
  • Ability to work both independently and collaboratively
  • Knowledge of local resources in the Greater Waterville area is a plus
  • Valid driver's license and reliable transportation
