Coordinates the pediatric wellness clinic (Victory) and coordinates appropriate management of pediatric wellness referrals. Provides nursing support through planning, assessment, implementation, evaluation, education and intervention among children, families, the schools and the communities of Winslow service area with the goal of preventing diabetes and other chronic disease illnesses such as obesity. Serves as the interim Youth Wellness Manager as needed. Uphold the principles of WIHCC’s Vision, Mission, and Value Statement. Maintains confidentiality of all privileged information at all times. This list of duties and responsibilities is illustrative only of the tasks performed by this position and is not all-inclusive.   Essential Duties & Responsibilities: Maintains regular attendance and punctuality. Performs nursing care in accordance with the nursing scope of practice and standards of nursing care across the age spectrum.  Documents patient encounters. Case manages and coordinates the care of pediatric patients who are at risk for developing type 2 diabetes and chronic diseases associated with obesity. Performs case management tasks related to identification and scheduling of patient’s needs. Leads pediatric wellness (Victory) case management meetings. Manages the scheduling of identified pediatric patients into the pediatric wellness clinic (Victory), diabetes clinic or other clinics as needed. Coordinates with the diabetes program to provide data for the annual SDPI Grant Audit. Plans, leads, and coordinates pediatric wellness clinic (Victory) group sessions. Provides outreach activities in collaboration with schools, WIHCC programs, tribal program, and community health programs in the Winslow Service Area targeting diabetes prevention. Coordinates the preparation of patient for tests, examinations and treatments. Maintains standards of care on patients coming to the pediatric wellness clinic (Victory). Performs clinical procedures when necessary. Coordinates care and patient visits with other departments for the prevention of high risk-related disease or illness of patients. Works in collaboration with Clinical Care Coordinators, Medical staff, Patient Access, Behavioral health, Nutrition department, Public Health Nursing for patient management and coordination. Provides health education to patients and family members and consults with the pediatric wellness (Victory) team members to determine the kind of care needed. Maintains health records by documenting observation, nursing interventions and therapeutic measures administered. Uses a community health assessment tool to assess the health needs of school age children within WIHCC service area. Serves as an advocate to the chapter communities to develop and implement community wellness initiatives for school age children. Uses the Dine Wellness Model as a framework to comprehensively address childhood obesity of the school age patient population for WIHCC: the individual, parent and family, school, community, and environment. Educates patients on diabetes self-management.  Provides consultation on diabetic risks, behavior risks (social and psychological) on one-on-one basis. Communicates and collaborates with School Health Specialist. Uses a quality assessment tool to assess, analyze, and interpret health information for school age children within WIHCC service area. Disseminates school health and wellness data to WIHCC clinical staff, stakeholders and community partners. Tracks budget accounts assigned to pediatric/Victory clinic activities. Integrates ethical provisions and leadership in all areas of nursing practice. Advocates protecting the health, safety, and rights of the population. Assist patients with initiating transportation services. Implements nursing care within the standards of care and the nursing scope of practice. Maintains proficiency and knowledge in trends affecting nursing issues such as treatments, concepts, skills, and nursing theories through continuing education, workshops, staff meetings, and Improving Organizational Performance team committees.  Requires completion of tasks or duties assigned by a supervisor. Upholds all principles of confidentiality and patient care to the fullest extent. Attends trainings, webinars, and meetings as required or requested. QualificationsMinimum Qualifications:   Bachelors of Science Degree in Nursing and five years of experience in community or public health nursing required. Must be licensed as a Registered Nurse in the state of Arizona (Excludes USPHS Commissioned officers). Current CPR certification is required. ACLS and PALS certification preferred. Experience in working with school age pediatric patients living with diabetes preferred. Certified Diabetes Educator certification preferred. Bilingual skills in English and Navajo language preferred.  Must maintain a valid unrestricted and insurable driver’s license.  Must successfully pass a background investigation and maintain suitable requirements for a Child Care position. This position is considered as a Child Care position, which requires a satisfactory background check investigation and is subject to the requirements of the Indian Child Protection and Family Violence Prevention Act, as amended (henceforth referred as the ICPFVP Act).   Knowledge, Skills, Ability Knowledge of professional conducts and upholds the nursing practice standards of care set forth by the state board of nursing. Knowledge and ability to apply principles, concepts, and theories of an evidenced based coordinated school health program with an emphasis on healthy behaviors to counter childhood obesity. Knowledge of and awareness of cultural and language differences. Will have frequent contacts with Navajo speaking stakeholders. Knowledge of communicable diseases and medical emergencies. Knowledge of established professional nursing principles, practice and procedures required to provide effective care and consultation regarding school health and school age children, management of type 1 and type 2 Diabetes Mellitus among youth, childhood obesity Knowledge of commonly prescribed pharmacological preparations used in the treatment and care of pediatric patients with diabetes, hypertension, and/or hyperlipidemia. Knowledge of and ability to independently assess the educational needs of an individual, family, and community Knowledge of established professional principles, practice and procedures required to provide effective care and consultation regarding pediatric obesity and diabetes. Knowledge of current Diabetes Management Clinical Guidelines and Standards: The Standards of Diabetes Care and Guidelines, the American Association of Diabetes and the Scope of practice, American Diabetes Association. Knowledge of current Pediatric Diabetes Standards of Care Guidelines and Standards. Familiar with Pediatric Chronic Disease illnesses Standards of Care and Guidelines and resources. Knowledge of documentation standards and ability to accurately and completely document clinical data relating to the patient and/or family. Knowledge of, and skill sufficient to recognize, deviations from normal sufficient to warrant immediate medication of the patient care plans, with delayed notification of the team leader for concurrence in the modification of the plan. Knowledge of health care delivery systems to effectively utilize available resources. Knowledge of strong work ethics in the workplace. Knowledge of basic application of confidentiality.
