An Excellent Opportunity

Mountain Valley Hospice is seeking a Registered Nurse (RN) Hospice Liaison to serve the High Point Service Area.

In this role, you will serve as the connection between the hospital and hospice—working closely with physicians, case managers, patients, and families to ensure a smooth, timely transition to hospice care when appropriate.

You will respond to hospice referrals, provide education, complete assessments, and facilitate admissions—often meeting patients and families during critical decision-making moments.

What This Role Looks Like

  • Maintain a consistent hospice presence within the hospital 
  • Partner with physicians, case managers, and hospital staff to support patient care planning 
  • Educate patients and families on hospice services and the end-of-life care journey 
  • Complete initial nursing assessments and facilitate hospice admissions 
  • Coordinate the referral-to-admission process, including documentation and care planning 
  • Perform hospice-in-place (hospital) admissions when appropriate 

This role requires strong clinical judgment, the ability to communicate clearly and compassionately, and comfort supporting patients and families facing serious illness and end-of-life decisions.

Schedule

  • Monday – Friday 
  • 8:00 AM – 5:00 PM 

Qualifications

  • Active Registered Nurse (RN) license 
  • BLS certification 
  • Valid driver’s license 
  • Experience in hospice, palliative care, or acute care preferred 
  • Strong communication skills and ability to work collaboratively in a hospital setting
