Requirements:

• Active, unrestricted Podiatry or Podiatric Medicine license in any U.S. state or Territory

• Doctor of Podiatric Medicine degree from an accredited educational institution

• Completion of an accredited podiatric residency program

• BLS (AHA or ARC)

• NPI

• Minimum of two (2) years of recent clinical experience in outpatient podiatry, ambulatory surgery, wound care, or a closely related practice setting.
