About the Role:
A Joyful Adult Health Care Center is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our caring team in Woburn, MA. Responsibilities:
- Provide direct patient care including wound care, medication administration, and vital sign monitoring
- Assist in developing and implementing individualized patient care plans
- Collaborate with RNs, physicians, and interdisciplinary care teams to ensure optimal patient outcomes
- Document patient assessments, treatments, and progress accurately in medical records
- Monitor and report changes in patient condition to supervising nursing staff or physicians
- Educate patients and families on health management, medications, and care procedures
- Maintain a safe, clean, and compliant clinical environment in accordance with state and federal regulations
Requirements:
- Active Licensed Practical Nurse (LPN) license in the state of Massachusetts
- Previous experience in adult health care, geriatrics, or a similar clinical setting preferred
- Strong knowledge of medication administration and infection control protocols
- Excellent communication and interpersonal skills with patients, families, and staff
- Ability to work effectively in a team-oriented, fast-paced environment
- CPR/BLS certification required or ability to obtain upon hire
- Compassionate, patient-focused attitude with a commitment to quality care
