What You'll Do:
Ā
TheĀ Case ManagerĀ IĀ facilitatesĀ admission intakes, treatment planning,Ā discharge planning, obtaining collateral andĀ assistingĀ in the legal processĀ andĀ initiatingĀ personalized care for individuals who are in crisis and stabilizing from crisis.Ā Works collaborativelyĀ withĀ theĀ interdisciplinaryĀ treatmentĀ team toĀ coordinateĀ careĀ andĀ dischargeĀ planningĀ forĀ Individuals, their family members, clinics, local providers, and/or other referral sources.Ā This position will carry a caseload and act asĀ ambassadorĀ to community partners.Ā
- Educates and informs individuals ofĀ consentsĀ for treatment and authorizations for use and disclosures.Ā
- ParticipatesĀ regularlyĀ in Interdisciplinary Treatment Team meetings.Ā
- Collaborates with the Treatment Team to develop and implement an individualized person-centered TreatmentĀ PlanĀ thatĀ isĀ appropriateĀ forĀ briefĀ intervention.Ā CompleteĀ theĀ signedĀ TreatmentĀ PlanĀ andĀ review with eachĀ individual andĀ any other involved parties.Ā Ā
- Gather collateral from theĀ individualĀ and involved partiesĀ to provide a thorough assessment of what anĀ individualāsāĀ needs are for treatment and discharge.Ā
- ActivelyĀ coordinateĀ careĀ and provide referralsĀ forĀ individualsĀ toĀ ensureĀ servicesĀ areĀ coordinatedĀ withĀ bothĀ facilityĀ staffĀ and external providers.Ā
- IdentifyĀ and address gaps in service needs for participating parties in service delivery to theĀ individual andĀ makeĀ appropriate recommendationsĀ toĀ meet IndividualsāĀ needs upon discharge.Ā
- CollaborateĀ withĀ InterdisciplinaryĀ TreatmentĀ TeamĀ toĀ engage,Ā monitor,Ā andĀ communicateĀ withĀ theĀ individual,Ā ensuring that safety isĀ maintainedĀ while meeting the individualās needs.Ā
- MaintainĀ theĀ clinicĀ record,Ā includingĀ documentationĀ ofĀ activitiesĀ performedĀ asĀ partĀ ofĀ theĀ serviceĀ delivery process.Ā Obtain signatures fromĀ individualsĀ regardingĀ treatment, as necessary. Document all services and IndividualsāĀ activities in the Electronic Health Record (EHR)Ā regardingĀ brief interventions, coordination of care, discharge planning, treatment planning,Ā and any other pertinentĀ information.Ā
- Maintain a working knowledge of theĀ VirginiaĀ Involuntary Commitment process.Ā Ā
- MaintainĀ aĀ workingĀ knowledgeĀ ofĀ theĀ DBHDS and DMAS behavioralĀ health system.
- ParticipateĀ inĀ individualĀ andĀ groupĀ supervisionĀ asĀ required.Ā ParticipateĀ inĀ ongoingĀ education,Ā includingĀ in-services, training, and other activities toĀ maintainĀ and improve competency.Ā Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.Ā
- Complete all required training, attend monthly meetings, andĀ maintainĀ required competencies.Ā
- Coordinates civil commitment process with all necessary stakeholders.Ā
- Provide daily programming/groups.Ā
- PerformsĀ allĀ otherĀ dutiesĀ asĀ assigned.
What You'll Bring:

A Bachelorās degree in behavioral health or related fieldĀ
-
Knowledge of the Commonwealth of Virginia behavioral health system of care and resources within it
- Qualified Mental Health Professional (QMHP)
- The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Companyās exemption process prior to their start date as a condition of employment
- State of Virginia Fingerprint Clearance
- Must be able to pass Virginia Department of Behavioral Health & Developmental Services (VA DBHDS) background check
- Must be able to pass Virginia Department of Social Services (VA DSS) Office of Background Investigations - Central Registry Search
It would be great if you had:

Masterās degree in behavioral health or related field

Licensed Mental Health Professional (LMHP)
-
Experience working with children, youth, families, and adults with behavioral health issues

Knowledge of and experience with the Virginia involuntary treatment process
