Symphony Applewood is seeking a compassionate and dedicated Social Service Director to support our short-term rehab and long-term care guests. This role is responsible for providing psychosocial support, discharge planning, advocacy, and care coordination while working closely with residents, families, and the interdisciplinary team.

Responsibilities

Conduct psychosocial and cognitive assessments
Develop and maintain resident care plans
Assist with discharge planning and transitions of care
Provide emotional support and advocacy for residents and families
Collaborate with nursing, physicians, and interdisciplinary team members
Maintain accurate documentation and participate in care conferences
Support end-of-life, hospice, and palliative care discussions

Requirements

Qualifications & Experience

Bachelor’s Degree required
Minimum 2–3 years of experience in long-term care and short-term rehabilitation preferred
Strong conflict resolution and problem-solving skills
Excellent communication and interpersonal abilities
Knowledge of federal and state regulations related to long-term care
Strong organizational and time management skills
Compassionate, professional, and empathetic demeanor
Knowledge of PointClickCare preferred
