Job DetailsJob Location: Operations and Resource Center - Woods Cross, UT 84010Intake / Admissions Nurse Responsibilities and Duties Receive information from referral sources and coordinate care for patients Verify and enter physician orders Identify level of care needed Receive and give verbal orders accurately Review clinical documentation to determine Skilled Nursing placement Exceptional interpersonal and customer service skills Ability to communicate clearly with pharmacist Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families Manage after-hours and weekend referrals (rotating on call) Schedule Monday – Friday (rotating shifts: 8:30-5:00pm / 10:00-6pm) One schedule weekend every 8-12 weeks, and rotating monthly on-call This position complies with the CMS Mandate for COVID 19 Vaccinations Job Type: Full-time Benefits: 401(k) Dental insurance Health insurance Life insurance Paid time off Vision insurance Standard shift: Day shift Weekly schedule: Monday to Friday Rotating weekends COVID-19 considerations: This position complies with the CMS Mandate for COVID 19 Vaccinations Ability to commute/relocate: Woods Cross, UT: Reliably commute or planning to relocate before starting work (Required) QualificationsQualifications: Current license as a licensed LPN/RN from the Utah State Board of Nursing 1-year Skilled nursing facility experience Good verbal and written communication skills Basic computer knowledge Ability to effectively problem solve Provide excellent customer service Knowledge of Medicare/Medicaid home care and hospice criteria preferred Knowledge of Medicare, Medicaid and 10A criteria
