About the Role

Are you a physician, nurse practitioner, or physician assistant who wants structured, meaningful clinical work without panel ownership or after-hours call? Thundermist Health Center is launching a Bridge Clinic at our Woonsocket location — a temporary initiative designed to provide interim primary care access for patients who don’t yet have an assigned PCP. You’ll deliver full-scope primary care during scheduled 30-minute visits, then hand off follow-up items to Thundermist’s cross-coverage team at the end of each shift. The schedule is fixed at three days per week, four hours per shift. No panel management, no after-hours call, no take-home work — just focused, patient-centered care for a community that needs it right now.

What You’ll Do

Bridge Clinic Clinical Care

  • Provide full-scope primary medical care to unassigned Thundermist patients during scheduled Bridge Clinic sessions
  • Manage acute care presentations, chronic disease exacerbations, medication management, and preventive care within 30-minute visits
  • Perform comprehensive health histories, physical examinations, and diagnostic assessments; order, interpret, and initiate treatment based on labs, imaging, and testing
  • Prescribe medications, including controlled substances, within scope of practice and in accordance with state and federal regulations
  • Triage patient concerns and escalate to higher levels of care (Convenient Care, ED, or specialty referral) when the Bridge Clinic model isn’t appropriate for the presenting concern
  • Provide clear discharge instructions so patients understand next steps and how ongoing care will be managed

Workflow & Handoffs

  • Operate within Thundermist’s defined Bridge Clinic workflow, including scheduled 30-minute visit blocks and end-of-day documentation completion
  • Route inter-visit follow-up items — test results, patient messaging, refill requests, and referral follow-up — to the cross-coverage queue for the broader clinical team
  • Communicate clearly with the cross-coverage team on any results, follow-up needs, or safety concerns from your visits
  • Coordinate with the Bridge Clinic support team (nurses, MAs, care managers) to ensure smooth patient flow and safe end-of-shift handoffs

Documentation & Compliance

  • Complete clinical documentation in the EHR (eClinicalWorks) accurately and by end of day to support continuity, billing, and quality reporting
  • Address care gaps, screenings, and preventive care within Bridge Clinic visits to support HRSA UDS reporting and value-based care performance
  • Maintain practice consistent with HRSA, OSHA, HIPAA, CLIA, and applicable state and federal regulations
  • Maintain unencumbered Rhode Island licensure, board certification, DEA registration, BLS/ACLS/PALS, and payer enrollment
  • Participate in Bridge Clinic peer chart review and quality improvement activities as scheduled

What We’re Looking For

You’re a provider who can walk into a room with a patient you’ve never met, quickly build rapport, deliver thorough care within a 30-minute window, and document cleanly before heading home. You’re comfortable working without continuity and trust the team behind you to carry the follow-up forward.

  • One of the following: M.D. or D.O. with completed residency in Family Medicine or Internal Medicine; NP with Rhode Island APRN licensure and ANCC/AANP certification; or PA with Rhode Island licensure and NCCPA certification
  • Active board certification in specialty required at time of hire
  • Full, unrestricted Rhode Island license to practice
  • Current DEA registration and Rhode Island Controlled Substance Registration
  • Current BLS certification; ACLS or PALS as applicable
  • Active payer enrollment or immediate credentialing eligibility with Medicare, Medicaid, and commercial payers (current CAQH profile required)
  • Working knowledge of electronic health records (eClinicalWorks preferred)
  • Comfort operating within a defined scope where inter-visit follow-up and cross-coverage handoffs are managed by the broader clinical team

Preferred:

  • Prior experience in an FQHC, community health center, or safety-net primary care setting
  • Prior per diem, urgent care, or locum tenens experience in ambulatory primary care
  • Prior experience with bridge clinic, transitional care, or unassigned-patient primary care models
  • Bilingual (Spanish/English or Portuguese/English) strongly preferred
  • Familiarity with team-based care models, integrated behavioral health, and SDOH screening

 

Schedule & Location

Per diem, hourly, non-benefited. This is a time-limited, temporary initiative. Fixed schedule of three days per week, four hours per shift (approximately 12 clinical hours per week), on-site at Thundermist’s Woonsocket location. Specific days and shift times are scheduled in advance by the Medical Director. Standard visit duration is 30 minutes. Documentation is completed by end of day. Test result follow-up and inter-visit messaging are routed to Thundermist’s cross-coverage queue — not carried by the Bridge Clinic Provider. No after-hours call, inpatient coverage, panel management, teaching, or precepting responsibilities.

Work Environment

  • Onsite at Thundermist’s Woonsocket location in an ambulatory clinical setting
  • Moderate physical activity: walking, standing, stooping, and occasional lifting
  • Potential exposure to blood-borne pathogens, respiratory disease, infectious diseases, and other biohazardous materials requiring universal standard precautions
  • Manual dexterity required for clinical procedures and equipment handling
