This position is a Part-Time 20 hours per week on site position to support the HIV Team.


Position Summary: The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services as part of the population health team.  The Community Health Worker will support the Care Management Team and the Population Health Team.

Essential Duties and Responsibilities:

•    Serve as contact-person for any new Hepatitis C Virus (HCV) patients

•    Enter new patients into the patient registry, which requires gathering information including Name, MRN, PCP, Insurance, fibrosis staging, genotype, consulting HCV provider

•    Stage patients by status code along the continuum of their care, which includes updating status codes with each transition of stage in evaluation and treatment

•    Collaborate with Pharmacy to ensure finding the correct payer for medications for patients being considered for treatment of HCV

•    Find and pre-fill all available information into the prior authorization form after completing a review of necessary lab and imaging data, as well as consulting provider’s note. 

•    Ensure patients have initial and follow-up appointments with HCV provider

•    Ensure patients have lab orders for 12 week post-treatment (SVR) testing. 

•    Manage all incoming faxes from insurance companies regarding prior authorizations

•    Contact insurance companies with any prior authorization clarification or denial.

•    Remind patients of upcoming appointments with the HCV treating provider.

•    Collaborate with UMass hepatology department to schedule fibro scan testing if indicated

•    Serve as point-person to patients along the continuum of HCV evaluation and treatment

•    Prepare for and attend weekly HCV team meeting

•    Check email and tasks once every HCV session for new consults or other communications.

•    Any and all other duties as assigned.

  Qualifications and Education Requirements

One of the following combinations of education and employment experience must be met in order to be considered for the position:

Education and Experience:

  • High school diploma or GED required; 
  • Bachelor’s degree is preferred.
  •  Preferred candidate will have Community Health Worker or Medical Interpreter certificate of completion.    
  •  Minimum of two years relevant work or college-level internship experience required. 
  •  Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners.

      •    Bilingual in English and another language appropriate to FHCW patient population required.

     •    Community Health Worker Core Training required within first year of employment.
