We are looking for full-time, independent, hard workers who love supporting those in need, while enjoying a flexible schedule, a supportive work environment, and competitive compensation. Our Community Support Program Case Managers work with adultsĀ 18+Ā in the community, living with all types of mental health needs. We provide skills training and connections to community resources to support independent living in their communities, decrease the risk of hospitalization or relapse of substance use, and improve overall quality of life.Ā

Essential Responsibilities:Ā Ā 

Ā 

  • Meet client weekly to provide outreach and support servicesĀ 
  • Engage client in the treatment system and support them to implement their goalsĀ 
  • Support client to attain skills and resources needed to maintain community tenure.Ā Ā Ā 
  • Coordinate Care with the providers/contactsĀ 
  • Provide telephone coaching and supportĀ 
  • Complete documentation of all meetings and contactsĀ 
  • Attend weekly staff/department meetings and supervisionĀ

Qualifications

  • Bachelor’s degree preferred in Psychology, Human Services, or a related field with one (1) year of experience providing direct careĀ 
  • A valid Massachusetts driver’s licenseĀ 
  • Daily access to a working, registered and insured vehicleĀ Ā 
  • Excellent in both written and verbal communicationsĀ 
  • Ability to independently plan and organize one’s own schedule and client servicesĀ 
  • Knowledge of Microsoft computer systemsĀ 
  • Strong time management skills and organization skillsĀ 
  • Bilingual applicants encouragedĀ 
  • Candidates must possess appropriate citizenship work documentationĀ 
  • Candidates must pass a CORI and drug test, as well as other pre-employment documentationĀ
