Care Coordinators are part of Wellness Now Clinic & Camp Hope programs. Wellness Now Clinic is an evidence-based model with a collaborative, cross-system approach to creative problem-solving. Care Coordinators work with clients to develop and implement individualized, cross-system care plans that work toward long-term goals. Care Coordinators must be skilled in managing team conflict, engaging a variety of personalities, facilitating meetings, and communicating with various audiences. Local travel is expected and this position will serve clients throughout Yakima County. Care Coordinator duties may include: Meet with eligible clients to assess needs, review social determinants of health, and develop individualized, cross-system care plans Identify barriers (housing, transportation, food, employment/financial, legal, childcare, medical, medication access) and connect clients to appropriate services Coordinate internal and external referrals, tracking progress from referral through service connection Conduct proactive outreach and re-engagement for clients who are difficult to reach or at risk of disengaging Facilitate communication and warm handoffs across care teams, providers, and community partners Document contacts, interventions, referrals, and outcomes in accordance with program standards Manage a caseload, monitoring progress toward goals and defined closure points Qualifications: Bachelor's degree in social work, psychology, or related field Must be eligible for registration as Agency Affiliated Counselor Must have valid driver's license and vehicle liability insurance In compliance with state and federal law, Comprehensive Healthcare requires all candidates to complete a post-offer, pre-employment drug screen and background check.
