Responsibilities

Provides optimal nutritional care to the hospital’s clientele, especially high risk/critical care patients, through assessing nutritional needs; developing, implementing, re-assessing nutritional care plans; and providing nutrition education/counseling to patients and family members. Performs nutrition screening to determine patients with nutritional needs. Performs nutrition assessments on patients identified as being at nutritional risk. Develops and implements nutrition care plans and goals for patients at nutritional risk. Reassess the effects of nutrition therapy, make relevant recommendations and provide continuum of care. Provides nutrition education/counseling to patients and family members. Demonstrates professional responsibility. Performs general departmental responsibilities. Maintains nutrition information documentation standards. Demonstrates organizational skills. Demonstrates communication skills. Demonstrates the ability to provide a safe and therapeutic environment for the patients, families and significant others, staff and visitors of the hospital. Performs other related assigned duties and performs unrelated duties in time of emergency. May be asked to work at more than one facility.

Qualifications

 

Bachelor of Science degree in course work approved by the Commission on Accreditation for Dietetics Education of the Academy of Nutrition and Dietetics. Proof of passing the registration examination administered by the Commission on Dietetic Registration (CDR). Must be in good standing with the Academy of Nutrition and Dietetics. Proof of maintaining professional licensure with the NewYork Education Department as a Certified-Dietitian-Nutritionist (CDN). Active membership in the Academy of Nutrition and Dietetics and/or the Association of Enteral and Parenteral Nutrition. Licensed dietitian status, if residing in a state with licensure requirements. Nursing Home and MDS experience. Knowledge of electronic medical record systems and computerized food service/menu systems.Recent clinical experience in an acute care hospital setting. Recent clinical experience in an acute care hospital setting as well as experience in long term care preferred.
