The RN Continuing Care Coordinator (CCC) is a RN Case Manager position which is responsible for cross continuum care coordination by facilitating patient care throughout the patient’s hospitalization, working with physicians, social workers and other health care team members to plan for continuing care needs beyond the hospital; overseeing the appropriateness of care provided to meet patient needs and regulatory/insurer requirements. The CCC works with physicians, peers, community colleagues, and others to orchestrate care across the health care continuum.

Required Experience:

  • InterQual experience, preferred
  • Three years required inpatient clinical experience in acute care setting involved with clinical activities and/or a managed care environment working in case management required
  • Excellent assessment, diagnostic and care planning skills
  • Two years recent experience in a case management role or related role preferred
  • Excellent computer skills
  • Demonstrates skills in working collaboratively with physicians, managers and other team members
  • Demonstrates skills in organizing and directing staff teams to complete assignments accurately and on time
  • Knowledge of community resources preferred

Requirements:

  • NYS RN License

Day Shift

NYSNA Base Rate $59.0359 additional compensation based on years of experience
