The Social Worker HealthySteps Specialist will be integrated into the primary care team to promote healthy emotional, social, and physical development in young children from birth to age 5. The HealthySteps Specialist works alongside physicians, nurses, and clinic staff to provide comprehensive, family-centered care focused on early childhood development, behavior, and family well-being.

The Social Worker will provide onsite mental health services, including psychosocial assessment, documentation of interventions/referrals made. Preferred experience working with at-risk families and experience in early childhood development, behavioral health, or family-centered services. Assist with implementation of treatment plans with patients. Work closely with medical team and staff to support referrals for services. Manage a caseload of patients.

ESSENTIAL DUTIES AND RESPONSIBILILITIES

The HealthySteps Social Worker will support families with young children by providing developmental guidance, screening, referrals, and parent support during well-child visits. The Social Worker will promote positive parent-child relationships, address behavioral and emotional concerns, and connect families to community resources.

  • In the pediatric primary care setting, the social worker will administer developmental and mental health screening tools and provide support and anticipatory guidance for common concerns for parents of children ages 0-3, including feeding, behavior, sleep, depression, and adapting to life with a baby or young child.
  • Conduct brief interventions for parents/caregivers, as clinically appropriate, such as on-site therapy sessions, for parents/caregivers with a positive depression screen or cases where the parent and/or the pediatric provider articulate a concern around mental health and parenting stress to the pediatric primary care team.
  • Participate in research, educational and advocacy initiatives related to Healthy Steps. Manage a caseload of patients. Participate in bi-monthly behavioral health conferences on high-risk patients.
  • Document psychiatric history and behavioral health assessment to develop patient care plan as indicated.
  • Offer information and counseling on the best course of action during sessions
  • Participate in care team meetings.
  • Responds with care team via EMR on patient progress or needs
  • Identifies community resources for the care of patients
  • Provide case management services to high-risk patients in collaboration with primary providers and other departments
  • Ability to build and maintain professional relationships
  • Ability to relate and communicate with diverse populations and groups
  • Resilience along with ability to assess situations
  • Maintain verbal and written communication with patients.
  • Attends training, workshops as requested

QUALIFICATIONS

  • Licensed LMSW or LCSW (Master's degree required )
  • A minimum of 2--3 years of experience in early childhood development, behavioral health, or family-centered services.
  • Experience working with children ages 0--5 and their caregivers in a medical, school, or community setting
  • Strong interpersonal, communication, and motivational interviewing skills.
  • Ability to work independently and collaboratively with cross functional teams.

PREFERRED SKILLS:

  • Preferred experience working with at-risk families impacted by mental health, substance use, and homelessness and knowledge of resources to support undocumented. Assist with implementation of treatment plans with patients
  • Bilingual English/Spanish preferred.

SCHEDULE

  • Monday to Friday 9am - 5pm; 1 evening 11 -7

1199 Salary Range: LMSW- $75,600 LCSW- $86,400
