This full-time position requires 80 hours biweekly, working Monday to Friday from 8:00 am to 4:30 pm, with some weekends and holidays in rotation.

Full Time (80 hours biweekly)

Monday - Friday 8:00am-4:30 pm.
Weekends and holidays rotation

General Summary

Performs activities and duties related to the education of the patient and referral source regarding services along the rehabilitation continuum. Completes and documents thorough preadmission assessments of patients to determine and recommend the most appropriate post-acute care setting to meet the patient's needs for maximizing independence and health and to support the seamless of transition of patients along the continuum.
