Windsor Home Health is looking for a compassionate and motivated PRN Occupational Therapist to join our growing Home Health team. We are committed to helping patients maintain independence, improve daily function, and safely remain in the comfort of their own homes.

Responsibilities

  • Evaluate patients' functional abilities and develop individualized treatment plans
  • Provide occupational therapy services in the home setting
  • Educate patients and caregivers on adaptive techniques and safety strategies
  • Collaborate with physicians, nurses, therapists, and interdisciplinary team members
  • Complete timely and accurate documentation
  • Monitor patient progress and adjust treatment plans as needed

Qualifications

  • Graduate of an accredited Occupational Therapy program
  • Current OT license in the state of Ohio
  • Home health experience preferred, but not required
  • Strong communication and organizational skills
  • Valid driver's license and reliable transportation
