Licensed Clinical Social Worker (LCSW)

Overview:
We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our care team. The LCSW will provide counseling, support, and care coordination to patients and families, addressing behavioral health needs, psychosocial challenges, and access to community resources. This role works collaboratively with physicians, nurses, and other healthcare professionals to improve overall patient well-being and outcomes.

Key Responsibilities:

  • Conduct psychosocial assessments and develop individualized care plans.
  • Provide counseling, crisis intervention, and supportive therapy to patients and families.
  • Collaborate with multidisciplinary teams to coordinate patient care.
  • Connect patients with community resources and social services.
  • Maintain accurate documentation in compliance with state, federal, and organizational standards.
  • Participate in quality improvement initiatives and case conferences.

Qualifications:

  • Master’s degree in Social Work (MSW) from an accredited program.
  • Current California LCSW license in good standing.
