You’ll help patients heal safely at home by delivering skilled, compassionate care and supporting the care team in developing individualized treatment plans. Key Responsibilities • Obtain medical history and assess patient conditions • Conduct physical assessments including vitals and mental status • Provide skilled nursing care per physician’s plan of care • Maintain patient confidentiality in compliance with HIPAA • Evaluate patient progress and communicate updates to the team • Submit all visit notes and documentation promptly • Collaborate with RNs and other care team members • Educate patients and families on care plans and home management QualificationsQualifications • Active LPN license in CA • Physical exam within the past 6 months • Current TB test (within 12 months) and CPR certification • Valid driver’s license and proof of auto insurance • Ability to attend a 2-day orientation at our Yuba City Locations We’re hiring in multiple states — AZ, CA, CT, FL, IL, IN, MA, MN, NV, and OH.
