We are seeking a compassionate and dependable Physical Therapist Assistant to join a post-acute care facility in Woodland, California. This role focuses on working with patients recovering from surgery, illness, injury, and medically complex conditions. Under the supervision of a licensed Physical Therapist, you will provide hands-on rehabilitative care aimed at improving mobility, strength, endurance, and overall functional independence. Job Responsibilities:
• Assist the Physical Therapist in implementing individualized treatment plans for post-acute patients
• Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions
• Monitor patient progress and communicate any changes or concerns to the supervising Physical Therapist
• Educate patients and caregivers on prescribed exercises, safety precautions, and mobility techniques
• Maintain accurate and timely documentation of treatments and patient progress
• Ensure compliance with facility policies, safety standards, and regulatory requirements





Requirements

• Active state license as a Physical Therapist Assistant in [STATE]
• Current BLS/CPR certification
• Minimum of 6 months of recent PTA experience within the last 5 years preferred
• Experience in post-acute, SNF, or rehabilitation settings is a plus
• Strong communication and teamwork skills
