Sage Group HealthCare Staffing is seeking LPN Home Health for multiple 13-week travel / contract positions in Zanesville, OH. Accepting both local and traveler candidates.

Pay Rate: $1680 Weekly Gross pay based on a 40 hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments
Unit: Home Health
Shifts available: Days
Position Requirements:
- LPN experience: 1 - 2 years - Required
- Recent/Current Home Health Nursing experience: 1 year within the last 3 years – Required
- Home Care Home Base (HCHB) charting experience – Required
- OASIS experience - Preferred
Education, License and Certification Requirements:
- LPN License for the state applying or Compact if accepted - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide and coordinate care for assigned Home Health patients under supervision of RNÂ
- Develop and implement nursing care plans for patients as assigned.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete chartingÂ
