This is a PRN position requiring flexibility with hours. There is no set schedule, and shifts will vary based on departmental needs.

Job Summary

Under the supervision of the Director/Manager of Pharmacy, the Clinical Pharmacist provides both distributive and clinical pharmacy services across the ambulatory infusion center and inpatient oncology units. Clinical Pharmacists play a vital role in the City of Hope care model, working directly with multidisciplinary teams in Medical Oncology and Surgical Oncology to deliver high‑quality, evidence‑based care and discharge counseling. Pharmacists follow hospital policies, practice agreements, and clinical guidelines to manage disease states, monitor chemotherapy toxicities, perform pharmacokinetic services, and ensure safe, cost‑effective medication use.

Responsibilities

  • Provide distributive and clinical pharmacy services in ambulatory infusion and inpatient oncology settings.

  • Collaborate with multidisciplinary teams, including oncologists, APPs, nursing, and other clinical staff.

  • Perform chemotherapy order verification to ensure accuracy, safety, and adherence to clinical guidelines.

  • Dispense medications and maintain compliance with regulatory and institutional safety standards.

  • Monitor patients for chemotherapy toxicities and participate in disease state management.

  • Provide pharmacokinetic dosing services and adjust therapy as clinically appropriate.

  • Conduct discharge medication counseling to support safe and effective transitions of care.

  • Deliver drug information and clinical guidance to providers and nursing staff.

  • Staff the ambulatory infusion center as needed, including assisting with infusion‑related medication workflows.

  • Participate in inpatient rounding and clinical decision‑making as part of the oncology care team.

  • Ensure safe, cost‑effective, evidence‑based medication use across all assigned areas.

Position Qualifications

Minimum Education

  • Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education.

Required Courses/Training

  • Completion of a pharmacy residency program preferred, or equivalent hospital pharmacy experience.

Required Certification/Licensure

  • Current Illinois State Registered Pharmacist license.

Preferred Experience

  • Experience in oncology pharmacy preferred.
