About the Role:

We are seeking a proactive and compassionate Social Worker to specialise in conducting Care Act assessments within our busy Locality Team. This crucial role involves:

  • Undertaking comprehensive needs assessments under the Care Act 2014 for adults requiring social care support.
  • Developing person-centred care and support plans in collaboration with individuals, their families, and carers, promoting independence and well-being.
  • Conducting Mental Capacity Act (MCA) assessments and acting in individuals' best interests where appropriate.
  • Working effectively within a multi-disciplinary team, including health professionals, housing, and other community services.
  • Managing a diverse caseload with varying levels of complexity, prioritising effectively and managing time efficiently.
  • Safeguarding vulnerable adults and responding to safeguarding concerns in line with local procedures and national guidance.
  • Providing advice and information to service users and their families on available resources and support options.


About You:

You will be a Qualified Social Worker with:

  • Demonstrable experience in adult social care, particularly with a focus on Care Act assessments.
  • In-depth knowledge of the Care Act 2014, Mental Capacity Act 2005, and other relevant adult social care legislation.
  • Excellent assessment, analytical, and risk management skills.
  • Strong communication and interpersonal abilities, capable of building rapport with diverse individuals and families.
  • The ability to work autonomously and as part of a supportive team.
  • Current registration with Social Work England.

Rate: 34.00 per hour (Umbrella)
