Key Responsibilities

  • Process incoming referrals, ensuring accurate data entry into the system.
  • Send correspondence to referrers and service users regarding referral outcomes.
  • Follow up with referrers to obtain missing information and occasionally take telephone referrals.
  • Build and maintain positive, empathetic relationships with families, therapists, and external professionals.
  • Liaise with the clinical lead to assess and determine referral suitability.
  • Provide information about the service and signpost to alternative support services when appropriate.
  • Escalate safeguarding or mental health concerns to the appropriate manager and follow internal protocols.
  • Maintain and update Excel trackers and case management systems.
  • Take minutes during meetings as required.
  • Manage and respond to emails in the service inbox.

Essential Skills & Experience

  • Excellent IT skills, including proficiency in Microsoft Office and case management systems.
  • Strong organisational skills with the ability to manage multiple tasks efficiently.
  • Confident, capable, and resilient under pressure.
  • Experience in a social care, education, or healthcare setting.
  • Good understanding of safeguarding principles and procedures.
  • Excellent communication and interpersonal skills.
