About the Role:Bristol City Council is seeking an experienced Social Workerto join our Adult Social Care Review Team. This is a fantastic opportunity to contribute to person-centred care planning and ensure services continue to meet the needs of adults across the city.
Working in a hybrid capacity, you’ll conduct strength-based reviews, working both remotely and face-to-face with service users, carers, and multi-disciplinary professionals.
Key Responsibilities:

Complete Care Act-compliant reviews of care and support plans

Promote independence and identify opportunities for care reduction where appropriate

Work in partnership with health professionals, providers, families, and advocates

Record assessments and updates in line with council policies and legislative requirements

Prioritise workloads effectively to manage a varied caseload
Requirements:
-
Qualified Social Worker with current Social Work England registration
-
Proven experience in adult social care, ideally within a review or assessment team

Sound knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding frameworks
-
Strong written and verbal communication skills
