We are seeking an experienced Qualified Social worker with the Adults Home First Team. Dedicated to facilitating efficient and effective hospital discharges. This is an excellent opportunity for practitioners with a passion for strength-based practice and experience in complex case management, including work related to the Court of Protection.
Key Responsibilities:
- Support hospital discharges by coordinating with healthcare providers and community resources.
- Conduct assessments and develop care plans that promote independence and well-being.
- Work collaboratively with individuals, their families, and multidisciplinary teams to ensure smooth transitions from hospital to home.
- Apply a strengths-based approach to support individuals in achieving their personal goals.
- Navigate complex cases, including those involving the Court of Protection, ensuring legal and ethical standards are met.
- Travel within the community to provide support and conduct assessments as needed
Requirements as a Social Worker include:
- SWE Registration
- Eligible to work in the UK
- Drive and have access to a car
- Post-qualified experience
- Degree level or equivalent in Social Work
