You will work directly with adults and their families, assessing needs, developing care plans, and providing support to enable individuals to maintain their independence, safety, and well-being within their own communities.Key Responsibilities:

  • Conduct comprehensive assessments of adult's needs, considering physical, emotional, and social factors.
  • Develop and implement person-centred care plans.
  • Work collaboratively with individuals, families, carers, and other professionals.
  • Advocate for the rights and needs of service users.
  • Facilitate access to appropriate services and resources.
  • Support individuals in their own homes and communities.
  • Maintain accurate and up-to-date case records.
  • Contribute to the ongoing development and improvement of the team's services.
  • Participate in regular supervision and training opportunities.
  • Comply with all relevant legislation and professional guidelines.

Essential Criteria:

  • A recognised Social Work qualification.
  • Current registration with the Health and Care Professions Council.
  • Significant post-qualifying experience in adult social work.
  • Excellent communication, assessment, and care planning skills.
  • Ability to work independently and as part of a multi-disciplinary team.
  • Full UK driving licence and access to a vehicle.

Desirable Criteria:

  • Experience working with specific adult populations (e.g., older adults, people with disabilities, mental health conditions).
  • Knowledge of relevant legislation and policy frameworks (e.g., Care Act 2014, Mental Capacity Act 2005).
  • Experience with case management software.
