Job Title: Adult Social WorkerHourly Rate: £19-£25 (dependent on experience)Location: [Specific Locality in Gateshead]Job Summary:We are seeking a highly motivated and experienced Adult Social Worker to join our dedicated Locality Team in Gateshead. You will work directly with adults and their families, assessing needs, developing care plans, and providing support to enable individuals to maintain their independence, safety, and well-being within their own communities.Key Responsibilities:

• Conduct comprehensive assessments of adult's needs, considering physical, emotional, and social factors.
• Develop and implement person-centred care plans.
• Work collaboratively with individuals, families, carers, and other professionals.
• Advocate for the rights and needs of service users.
• Facilitate access to appropriate services and resources.
• Support individuals in their own homes and communities.
• Maintain accurate and up-to-date case records.
• Contribute to the ongoing development and improvement of the team's services.
• Participate in regular supervision and training opportunities.
• Comply with all relevant legislation and professional guidelines.

Essential Criteria:

• A recognised Social Work qualification.
• Current registration with the Health and Care Professions Council.
• Significant post-qualifying experience in adult social work.
• Excellent communication, assessment, and care planning skills.
• Ability to work independently and as part of a multi-disciplinary team.
• Full UK driving licence and access to a vehicle.

Desirable Criteria:

• Experience working with specific adult populations (e.g., older adults, people with disabilities, mental health conditions).
• Knowledge of relevant legislation and policy frameworks (e.g., Care Act 2014, Mental Capacity Act 2005).
• Experience with case management software.
