- Conduct comprehensive needs assessments of adults, identifying support requirements and developing person-centered care plans.
- Work collaboratively with individuals, families, carers, and other agencies to coordinate services and ensure holistic support.
- Advocate for the rights and needs of adults within your caseload, promoting their independence and well-being.
- Manage a diverse caseload, balancing competing priorities and ensuring timely interventions.
- Contribute to the ongoing development and improvement of services within the South Locality.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience working within a locality team is desirable.
- Excellent assessment, communication, and case management skills.
- Strong understanding of relevant legislation and policies relating to adult social care.
- Ability to work independently and as part of a team.
- Commitment to person-centered practice and ethical standards.
