This is a rewarding opportunity to support adults within a specific geographical area, making a real difference to their lives.About the Role:

  • Conduct comprehensive needs assessments of adults, identifying support requirements and developing person-centered care plans.
  • Work collaboratively with individuals, families, carers, and other agencies to coordinate services and ensure holistic support.
  • Advocate for the rights and needs of adults within your caseload, promoting their independence and well-being.
  • Manage a diverse caseload, balancing competing priorities and ensuring timely interventions.
  • Contribute to the ongoing development and improvement of services within the South Locality.

Requirements:

  • Registered with Social Care England.
  • Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience working within a locality team is desirable.
  • Excellent assessment, communication, and case management skills.
  • Strong understanding of relevant legislation and policies relating to adult social care.
  • Ability to work independently and as part of a team.
  • Commitment to person-centered practice and ethical standards.
