- Developing and maintaining individual support plans and risk assessments
- Liaising with a variety of partner agencies, including mental health services, housing providers, benefit agencies, and training/employment organisations
- Promoting independent living, helping clients achieve their personal goals and aspirations
- Supporting clients with complex challenges, including mental health issues and substance misuse
- Taking a positive approach to risk management
- Previous experience supporting vulnerable individuals (D)
- Understanding of housing-related support (D)
- Ability to work both independently and as part of a team
- Excellent communication and relationship-building skills
- Strong organisational and problem-solving abilities
- Flexibility to meet the needs of clients, including occasional work outside normal hours
- A clear understanding of safeguarding, diversity and promoting independence
- A good level of literacy and numeracy
