We’re looking for a forward-thinking Primary Care Transformation Manager to join our client’s team at Rosedean House Surgery. This role is an exciting opportunity to help shape the future of healthcare delivery, enhancing patient care and modernising our GP practice.

As our Primary Care Transformation Manager, you'll lead strategic projects that improve access, streamline services, and ensure high-quality, patient-centred care. You’ll collaborate with practice teams, local Primary Care Networks (PCNs), Integrated Care Boards (ICBs), and other key stakeholders to bring about sustainable, impactful change for our patients.

What You’ll Be Doing

  • Lead transformation initiatives that enhance patient access, service efficiency, and clinical outcomes
  • Translate strategic goals into actionable improvements at the practice level
  • Drive innovation in general practice, implementing modern initiatives aligned with NHS priorities
  • Ensure the practice is ready for CQC inspections through audits and staff training
  • Work alongside clinical and non-clinical teams to embed continuous improvement and sustainable change
  • Oversee performance on NHS income streams such as QoF, IIF, LES/DES
  • Enhance patient engagement, contributing to the growth of the practice
  • Foster strong relationships with stakeholders and promote our client within the community
  • Provide day-to-day leadership and support for the practice team

What We’re Looking For

  • Proven experience in Primary Care management and transformation
  • Strong knowledge of NHS structures, GMS contracts, and Primary Care operations
  • A leadership style focused on developing people and services
  • Ability to manage complex change while maintaining operational excellence
  • Collaborative, innovative mindset with a focus on outcomes and quality
