Our client is seeking an enthusiastic Occupational Therapist to join a Integrated Discharge Service (IDS) — a fast-paced, multi-disciplinary team delivering safe and effective hospital discharges, 7 days a week. This is a rewarding opportunity to make a real difference to the lives of residents as they transition from hospital back to their homes and communities.

Your Key Responsibilities

  • Support Discharge to Assess (D2A) pathways by screening referrals and planning timely, appropriate interventions for hospital discharges.
  • Undertake post-discharge home assessments to deliver personalised therapy interventions that promote independence, support recovery, and reduce readmission risk.
  • Design reablement plans and set client-centred goals based on strength-based approaches and meaningful outcomes.
  • Assess and mitigate risks in the home environment, including the provision of equipment, minor adaptations, and safe manual handling strategies.

What We're Looking For

  • HCPC-registered Occupational Therapist.
  • Strong experience in hospital discharge or community-based rehabilitation.
  • Confident clinical decision-maker with excellent communication and organisational skills.
  • Committed to strength-based practice and empowering service users to reach their goals.
