Job Overview:As a Band 6 Occupational Therapist, you will be responsible for delivering comprehensive therapy assessments, interventions, and discharge planning for patients in various community settings. You will be working closely with patients and other healthcare professionals to ensure high standards of care and positive patient outcomes.

Key Responsibilities:

  • Conduct patient-facing therapy assessments, risk assessments, and clinical reasoning to develop tailored intervention plans

  • Set therapy goals and plan interventions to support patient recovery and independence

  • Plan and coordinate safe and effective discharges

  • Prescribe, review, and monitor equipment to meet patients' needs

  • Work autonomously and effectively in a community setting, including urgent and rapid response environments

  • Utilize EPIC and other electronic health record systems for documentation and care planning

  • Comply with the standards set by the Health & Care Professions Council (HCPC) and other relevant professional bodies

Requirements:

  • Professional registration with the Health & Care Professions Council (HCPC)

  • Experience in urgent/rapid response and community-based settings

  • Competence in risk assessments, clinical reasoning, and lone working

  • Proficiency in using EPIC or similar electronic health record systems

  • Excellent communication, time management, and teamwork skills
