We are currently seeking an experienced Social Worker to join the Front Door team in Waltham Forest. This role is central to the initial point of contact for Adult Social Care, focusing on effective triage, screening, and timely interventions.
Key responsibilities:

Undertaking Care Act assessments and proportionate enquiries

Managing safeguarding concerns and escalating appropriately

Working collaboratively with health and community partners

Ensuring person-centred support planning and signposting to appropriate services

Supporting individuals with a wide range of needs including older adults, those with physical disabilities, and mental health concerns
Requirements:

SWE registration
-
Experience within a front door or community access team
-
Strong knowledge of the Care Act, MCA, and safeguarding procedures

Excellent communication and decision-making skills
