Role OverviewAs a Social Worker within the Transformation Older Person Mental Health Team, you will play a pivotal role in delivering high-quality, person-centred care to older adults experiencing mental health challenges. Your responsibilities will encompass comprehensive assessments, care planning, and collaborative interventions aimed at enhancing the well-being and independence of service users.
Essential Qualifications & Experience-
Professional Qualification: Degree in Social Work or equivalent.

Registration: Current registration with Social Work England.

Experience: Minimum of 2 years post-qualification experience in adult social work, with a focus on mental health and older adults.

Legislative Knowledge: In-depth understanding of relevant legislation, including the Mental Health Act, Mental Capacity Act, and Care Act.

Communication Skills: Excellent verbal and written communication abilities, with the capacity to engage effectively with service users, families, and professionals.

Safeguarding Awareness: Knowledge of safeguarding procedures and the ability to identify and address potential risk

Comprehensive Assessments: Conduct thorough mental health and risk assessments to identify the needs of older adults, utilizing evidence-based tools and frameworks.

Care Planning: Develop and implement individualized care plans that promote recovery, independence, and quality of life, in line with the Care Act and Mental Capacity Act.

Multi-Disciplinary Collaboration: Work closely with a range of professionals, including healthcare providers, occupational therapists, and community services, to ensure holistic support for service users.

Safeguarding: Identify and respond to safeguarding concerns, ensuring the protection and well-being of vulnerable adults in accordance with statutory requirements.

Community Engagement: Facilitate access to community resources, support networks, and social activities to combat isolation and promote social inclusion.

Crisis Intervention: Provide timely and effective interventions during periods of crisis, offering support and guidance to service users and their families.

Documentation and Reporting: Maintain accurate and up-to-date records of assessments, care plans, and interventions, adhering to professional standards and organizational policies.
