Responsibilities
  • Fulfill a social work role in a multi-disciplinary setting, collaborating across professional boundaries.
  • Assess and develop clear and effective care plans in partnership with clients, their families, and other interested parties.
  • Support clients and their carers during periods of major changes and stress, respecting diverse cultures, backgrounds, religions, and circumstances.
  • Develop positive working relationships with internal and external partners, such as Health, Commissioning, and Housing.
Person Specification
  • Experience in a social work role within a multi-disciplinary setting.
  • Ability to create effective care plans in collaboration with clients and stakeholders.
  • Competence in working with diverse populations, providing support during significant life changes.
  • Proven track record of building strong partnerships with various internal and external agencies.
