You will play a crucial role in supporting vulnerable adults to live independently and safely in their own homes and communities.Key Responsibilities:Carry out strengths-based assessments and person-centred care planning.Work within the Care Act 2014 framework to deliver statutory adult social care services.Manage a varied caseload including complex cases, safeguarding concerns, and mental capacity assessments.Collaborate effectively with health professionals, voluntary organisations, and other partner agencies.Support hospital discharge planning, reablement, and prevention of admission.Record casework accurately on the council's systems and contribute to performance targets.Requirements:Qualified Social Worker (BA, MA, DipSW or equivalent).Current registration with Social Work England.Minimum of 1 year of experience in adult social care (local authority preferred).Good working knowledge of legislation such as the Care Act, Mental Capacity Act, and safeguarding adults procedures.Enhanced DBS (Disclosure and Barring Service) check - registered or willing to apply.Ability to travel within Northumberland as required.Benefits:Competitive hourly pay (£20-£26, depending on experience).Weekly payroll via umbrella or PAYE.Supportive team environment and flexible working opportunities.Access to supervision, training, and CPD opportunities.
